Required Documents/Information

ADULT APPLICATIONS: 

• Completed DS-11 Passport Application, completed in black ink  (No double-sided applications)  DO NOT SIGN
• Passport photos (Must be recent, in color, and do not wear glasses in photo)
• Proof of identity (valid Virginia driver's license)
• Proof of U.S. Citizenship (Original birth certificate with seal and both parents listed or expired adult passport)
• Copy of driver's license (front & back on separate sheets) & copy of birth certificate (black & white copy)
• Check or Money Order to send with Passport Application
• Separate payment for Execution fee of $35.00 per application (check, money order, cash, credit card (convenience fee applies))

MINOR APPLICATIONS:

• Completed DS-11 Passport Application, completed in black ink  (No double-sided applications)  DO NOT SIGN
• Minor must be present
• Both parents must be present with proof of identity (Valid Virginia driver's license).  If one of the parents cannot be present, they must complete a form DS-3053 & submit a copy of the front & back of their driver's license.
• Passport photos (Must be recent, in color, and do not wear glasses in photo)
• Proof of U.S. Citizenship (Original birth certificate with seal and both parents listed)
• Copy of parent's driver's license (front & back on separate sheets) & copy of minor's birth certificate (black & white copy)
• Check or Money Order to send with Passport Application
• Separate payment for Execution fee of $35.00 per application (check, money order, cash, credit card (convenience fee applies))
